21-year-old male, nephrotic syndrome with proteinuria of 6.6g in 24 hours.

The glomerular capillary slings are thickened and show silver staining with the Silver-methenamin spike stain on the outer side and holes of the basal membrane. The findings affect all glomeruli to an equal extent.

The light microscopy findings of spikes correspond to the sawtooth-like basal membrane reaction which occurs next to the regularly arranged immune complex deposits on the outer side of the glomerular basement membrane, which are only visible with certain special methods.
