64year-old male, frank haematuria. Nephrectomy
- A solid carcinomais located in the adipose tissue of the hilum.
- In better differentiated sections a similarity with dysplastic urotheliumcan be seen.
- Other parts contain less differentiated infiltrative anaplastic tumour cells.
- In between, there is a gradual change from the relatively well to poorly differentiated sections of the tumour (phaenotypic tumour heterogeneity)
Grading: The urothelium carcinomas are classified according to degree of differentiation. According to least differentiated parts the grading here would be grade III.
Staging: The depth of infiltration which, especially in the bladder, is of therapeutic and prognostic significance and cannot be provided precisely in this specimen according to scheme (pTX), an infiltration of the hilar adipose tissue is extremely likely.
