50-year-old female, mechanical cholestasis with carcinoma of the pancreas. elevated aminotransferases to 3-4 times the norm (AST 40IU/I, ALT 45 IU/I) markedly elevated alkaline phosphatase (650 IU/l) and Gamma GT (220IU/I). Billirubin 10 times the norm (180IU/I)

- Plenty of green to blue/green, partly drop shaped, partly branched biliary thrombi
- Inspissated bile in the distended canaliculi (canalicular cholestasis)
- Bile drops in the cytoplasm of the hepatocytes (retention)
- Bile in Kupffer cells (phagocytosis)
- Individual hepatocytes show mesh-like loosened cytoplasm (degeneration caused by accumulated bile salts)
- Portal field with fibrosis; enlarged with numerous proliferated bile canaliculi, especially at the edges.
