60-year.old male, death in hepatic coma following decompensated cirrhosis. AST 120 IU/l, ALT 80IU/l. Profound icterus. Gamma GT 320 IU/l (for reference ranges refer to specimen SII3 and SII6)

- Advanced liver cirrhosis
- Peripheral pseudolobules (no central nerves visible)
- Clusters of steatosis (moderate sized drops)
- Mallory bodies in ballooned hepatocytes: fresh ones in blue, old ones with bright red centre and blue core
- Extensive cholestasis with bile in the distended canaliculi as well as in some thickened ductules and pseudoductules
- Individual (mainly ballooned) hepatocytes with highly enlarged mitochondria (bright red balls). Extensive mesh fibrosis. Shunt vessels in the connective tissue septa (often portovenous). These short cuts - together with the extrahepatic shunts - can lead to hepatic coma.
