The spleen is pervaded by numerous small caseous necrotic areas. Some of these are not surrounded by an epitheloid cellular reaction. Others are delimited by few epitheloid cells and an occasional multinucleated giant cell.
Pathogenesis: Hematogenous dissemination of tubercle bacilli in patients with impaired immunity.
