72-year-old male underwent mediastinal radiation therapy because of small cell lung cancer 2 years before death (specimen from autopsy).
- A section through the wall of the left ventricle is shown.
- Marked endocardial and subendocardial fibrosis and elastosis is present.
- Interstitial and perivascular fibrosis of the myocardium is observed. Myocytes display enlarged nuclei as a sign of hypertrophy. The capillary vessels show a marked shift in size distribution with presence of capillaries exhibiting large diameters (teleangiectasies caused by radiation). In addition, blood vessels show elastosis and some arteries intimal fibrosis with partial occlusion of lumens. Some endothelial cells exhibit slightly enlarged polymorphic and hyperchromatic nuclei.
- The visceral pericardium shows fibrosis and focal fibrinous deposits
Fibrosis affects all layers of the wall; collagen Type I is primarily identified.
